Description of Services:	Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups Community-led health teams develop programs to improve the social supports and conditions that affect long-term health Primary Health Care Services - Health care team includes doctors, nurse practitioners, registered nurses, dietitian, physiotherapist, kinesiologist, social workers, chiropodist (foot care specialist), health promoter and community developer * in-home palliative care services Chronic Disease Self-Management - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions (open to everyone) Smoking Cessation - One-on-one support to help quit smoking (open to everyone) Free Seasonal Flu Vaccines - Available for all community members * drop in and bring Ontario Health Card GB Works Needle Syringe Program - needles and other injection drug supplies provided and accepted for disposal * offered in partnership with Grey Bruce Health Unit Free Tax Preparation Clinic - Offered through the Community Volunteer Income Tax Program of Revenue Canada * locations in Chatsworth, Dundalk, Durham, Flesherton, Holstein, Markdale Other community services: support provided to help fill out government forms * call for appointment free phone and computer/internet access ride share program community volunteer opportunities   2S-LGBTQQIAP+ Connection Drop-in meetings for networking, conversation, and informal support for the Lesbian, Gay, Bisexual, Trans, Queer community, and families, friends, and allies   Breastfeeding Support Breastfeeding support and assistance by a certified lactation consultant * one-on-one appointments in the clinic or telephone support available   Community Food Programs Community food programs for all members of the community * programs vary based on time of year and community needs * wellness, nutrition and cooking classes   Counselling Services One-on-one, couple, and family counselling * mental health screening and assessments * short-term and long-term counselling * confidential   Dundalk Erskine Site Community health centre offers services for all ages including community programs and social work, physiotherapy, dietitian and kinesiology services   Fitness and Recreation Programs Provides a variety of fitness and recreation programs for community members   Foot Care Clinic Chiropodist (foot care specialist) and nurses assess and treat foot problems * service is provided to people who are at a higher risk of foot complications, such as people with diabetes, peripheral vascular disease, and immune-compromised   Medical Equipment Service Provides free used medical equipment and mobility aids in good condition such as walkers, canes, commodes, crutches * equipment can be kept or donated back * accepts new and used equipment from the community * waiting list kept for equipment requests   Nutrition Counselling Nutritional information and education * one-on-one, couple, or family sessions for people with food and health-related concerns Services and Programs: Chatsworth Site Dundalk Medical Centre
