Description of Services: Integrated stroke care * information and resources for stroke survivors * promotes healthy living and stroke prevention with community partners

Care provided from the emergency room and inpatient setting, to rehabilitation and return to the community * works with partners in long-term care
 
Integrated Stroke Unit 13-bed acute and rehabilitative care program for people who have had a stroke * 5 acute care beds and 8 rehab care beds * services provided by doctors, nurses, personal support workers, physiotherapists, occupational therapists, speech language therapists, dietitians, pharmacists, and social workers
 
Stroke Prevention Clinic Stroke prevention for patients within their community, who are at high risk for stroke, have had a Transient Ischemic Attack (TIA) and/or who have had a non-disabling stroke
