Description of Services: Assistance for adults returning home from a hospital stay

Up to 3 hours of care on day of discharge * includes transportation from the hospital, pick up of groceries, medical equipment, medication, home management such as meal preparation, light housekeeping, laundry and follow-up call 
Requirements
Fees: Free for those who meet eligibility criteria
Eligibility - Population(s) Served: People over 55, or younger people with complex needs who need assistance when being discharged from hospital
Application: Self referral to VON or by contacting Ontario Health atHome South West area office for those who meet eligibility criteria
Accessibility Notes: Service provided from hospital to client's home in the community
