Description of Services: Multi-agency team that provides access to mental health services including information and referral, mental health counselling, intensive case management, housing and outreach support, group services, social recreational activities and leisure access support services

Outreach Community Support - Support program for adults with serious mental illness living in the community. Supports people with:
  • Housing options available
  • Daily living skill development
  • Supportive counselling
  • Linking with other formal and informal supports
  • Advocacy social/recreational enrichment
Requirements
Fees: Free * fees for some social/recreational activities * rent subsidy paid by clients who are housed within the apartment program
Eligibility - Population(s) Served: People 16 years and older with a serious mental illness and their families
