Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, social workers, pharmacists, dietitians, respiratory therapists, and occupational therapists

patient access during extended evening and weekend hours for urgent problems
after-hours telephone assistance
assistance in navigating the health care system
chronic disease management
health promotion and education

South West Self-Management Program Help for people living with chronic conditions who want to make healthy changes and manage their illness * support for caregivers * professional development opportunities for service providers
