Description of Services: Offers a range of programs and services

Services offered:
  • well water sample kit pick-up and sample drop-off
  • Oral health services
 
Adult Day Services Adult day program * social, recreational, and therapeutic activities for older adults, adults with special needs, and adults with Alzheimer's disease and other related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Assisted Living Provides on-site personal support services to eligible individuals
 
Exercise and Wellness Programs include blood pressure checks (CHAP - Cardiovascular Health Assessment Program), health education, group exercise classes, in-home one-on-one exercise programs * held in various locations * view agency website for locations and times
 
Foot Care Basic and advanced foot care provided by nurses with a certification in advanced foot care at community clinics
 
Friendly Visiting and Telephone Reassurance Regular volunteer visits to older adults and people with disabilities who live alone or are identified as someone who would benefit from this program * volunteers are matched one to one with older people
 
Home At Last Program Assists older adults with transitioning from hospital to home
 
Home Help Assistance with household chores and daily tasks
 
Meals on Wheels Meal delivery is offered out of various locations throughout Huron and Perth * call for further information
 
ONE CARE Lifeline Emergency Response Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
 
Personal Support and Homemaking Services Assistance with daily chores, such as laundry, light housekeeping, pet care, errands, shopping, and meal preparation * service providers visit weekly, bi-weekly, monthly, or seasonally
 
Respite Overnight relief for caregivers of older adults, frail elderly and those with Alzheimer's disease and related dementias * accommodations are offered for either one or two nights at the Wingham and Goderich Adult Day Centres from 10am Thu to 3pm Sat * trained staff provide assistance with activities of daily living, personal care and structured activities in a safe environment
 
Transportation - EasyRide Non-emergency transportation to local medical appointments and regular activities such as personal errands and social activities * volunteer drivers * wheelchair accessible vehicles available
 
Wingham Dining for Seniors Group meal, social activities, entertainment and education * call in advance for information and to register
