Description of Services:	Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * Wellness for Seniors programs include exercise, health screenings, meals and foot care * referral to other community supports referral to other community services   Accessible Tub and Shower Room Whirlpool tub and electronic lift available for people who require accessibility supports. There is a fee of $15/hour and client is responsible for own support person if required. Hours are by appointment only.   Blood Pressure Program As part of our Wellness Program, our Blood Pressure Program's goal is to promote cardiovascular health, healthy lifestyles, personal empowerment, and regular blood pressure monitoring for participants.   Meals on Wheels Hot meals delivered Mon-Fri 4:30pm-5:30pm to participants living within the community of Zurich * meals include a salad, entree, and dessert * special diet accommodations diabetes-friendly options are available upon request   Wellness for Seniors Wellness programs, including education, exercise and falls prevention and health screening for adults who are 55 years of age and older
