Description of Services: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
 
Home At Last Identifies older adults in local hospitals who have limited supports upon discharge
 
Home Help Program Assistance with daily chores, such as laundry, light housekeeping, errands, grocery shopping, and meal preparation * home helpers visit weekly, bi-weekly, monthly (every 4 weeks)
 
SMART Falls Prevention and Group Exercise Classes for Seniors Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls
 
Volunteer Visiting Support program that matches volunteers with a senior or adult with a disability for one-on-one visits in the client's home or a weekly chat over the phone * visits are regular and may also include enjoyable activities outside of the home
 
Wellness Link Programs to support independent living for seniors and adults with disabilities are offered at London Housing Corporation rent-geared-to-income apartments
