Eligibility - Population(s) Served: | Physically disabled adults who are able to direct their own care * must be 16 years and older with a permanent physical disability or acquired brain injury * require assistance with personal care and activities of daily living * have the ability to direct own care * be a resident of Ontario with a valid OHIP * qualify for Aging at Home program * not be in receipt of insurance funds | ||
