Organization: ONE CARE Home and Community Support Services
Description of Services: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
 
Assisted Living On-site personal support services for eligible individuals *  helps with daily activities and supportive services to maintain independence * a hub model providing service to the community as well as designated buildings * sites include Maplewood and Birchwood Apts in Zurich
 
Dining for Seniors Hot meal and conversation for seniors
 
Exercise and Wellness Focuses on seniors maintaining and/or improving their overall health
 
Foot Care Basic and advanced foot care provided by nurses certified in advanced foot care management at community clinics for people with diabetes, older adults and adults with a disability
 
Friendly Visiting and Telephone Reassurance Regular volunteer visits to older adults and people with disabilities who live alone or are identified as someone who would benefit from this program * volunteers are matched one to one with older people
 
Home At Last Program Assists older adults with transitioning from hospital to home
 
Home Help Assistance with daily chores, such as laundry, light housekeeping, heavy cleaning, yard work, errands, shopping, and light meal preparation * Home Help workers' visits can be done working with the client's schedule and preferences
 
Lifeline Emergency Response Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
 
Meals on Wheels Hot meals delivered Mon-Fri between 11:30am-1pm * special diet types available
 
Overnight Respite Overnight relief for caregivers of older adults, frail elderly and those with Alzheimer's disease and related dementias * accommodations are offered for either one or two nights at the Wingham and Goderich Adult Day Centres * trained staff provide assistance with activities of daily living, personal care and structured activities in a safe environment
 
Personal Support and Homemaking Services In-home supports for the activities of daily living * personal care support worker helps with personal care
 
South Huron Adult Day Program Adult day program * social, recreational, and therapeutic activities for older adults, adults with disabilities, and people with Alzheimer's disease and related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Transportation - EasyRide Non-emergency transportation to local and medical appointments and regular activities such as shopping, errands, or social activities * volunteer drivers * wheelchair accessible
