Description of Services: Multi-agency team that provides access to mental health services including information and referral, mental health counselling, intensive case management, housing and outreach support, social recreational activities and leisure access support services

Outreach Community Support - Support program for adults with serious mental illness living in the community. Supports people with:
  • Housing options
  • Daily living skills development
  • Supportive counselling
  • Linking with other formal and informal supports
  • Advocacy social/recreational enrichment
Requirements
Fees: Free * fees for some social/recreationall activities; subsidized housing rates paid by clients of the apartment program
Eligibility - Population(s) Served: This program is for young adults 16 years of age and older who:
  • reside in Hanover, Markdale, Kincardine, Southampton or the surrounding area
  • have the willingness and a commitment to engage with your counsellor
